Phase Overview
In this training phase, Family Navigators will focus on providing necessary employment onboarding documentation. Navigators will also begin to develop a general understanding of School-based Mental Health and Wellness, recognizing the intersection and integration between healthcare and education systems and the impact on the larger community.

Complete General Forms prior to Orientation
Complete the forms provided below to help prepare for your upcoming orientation. This will ensure that the introduction to your role is well-organized and runs smoothly. Your cooperation will contribute to a productive and successful orientation experience.

Employment Onboarding Requirements
Ensure completion of all required HR tasks, including background checks, submission of identification and health documentation, and provision of necessary forms and photos. This comprehensive process ensures that all necessary credentials and clearances are obtained for the Family Navigator role.

• Fingerprinting for Health Center and SCOE
• Resume
• Government-Issued Photo ID
• Documentation of current PPD results (TB test)
• Documentation of current Flu Vaccination or sign the Flu Declination Form, and abide by policy
• Molina HIV and AIDS Specialist Form
• Documentation of COVID-19 Vaccine
• Submit appropriate photo of self for SCOE Badge
Onboarding Support
Understand who to contact for support during the onboarding process. Family Navigators are given specific administrative points of contact within the program to facilitate efficient communication and support. These contacts are essential for addressing administrative needs and obtaining necessary information.

• Meet with SCOE personnel and payroll contacts to complete all necessary paperwork
• Coordinate with administrative staff to retrieve technology (phone, computer, fob, lock box, badge)
